Two EMA authorizations: (1) Ozempic — semaglutide 0.5/1/2 mg injection, authorized 2018 for adults with insufficiently controlled type 2 diabetes mellitus; (2) Wegovy — semaglutide 0.25/0.5/1/1.7/2.4 mg injection, authorized 6 January 2022 for weight management in adults with BMI ≥30, or BMI ≥27 to <30 with weight-related comorbidity, also approved for adolescents ≥12 years with obesity and weight >60 kg. Ozempic is NOT authorized for weight management — Wegovy is. Off-label use of Ozempic for weight loss is legally permissible but not GKV-reimbursed.

Prescription reality

Any licensed German physician can prescribe Ozempic for type 2 diabetes or Wegovy for weight management (within the BMI/comorbidity criteria). Off-label prescribing of Ozempic for weight loss is legally permissible but requires documented physician judgment and is not GKV-reimbursed.

Compounding pharmacy nuance

Not relevant for legitimate trade — finished EU-authorized products exist. Compounded semaglutide products from non-EU sources are unauthorized and unsafe.

Research-use legal basis

Not applicable. Semaglutide is a finished medicinal product. Research-grade vials sold as 'semaglutide for research use' are typically counterfeit or grey-market diverted product.

Pharmacy availability: Both Ozempic and Wegovy available at licensed German Apotheken on prescription. Wegovy supply has been variable since launch but improved by 2024.

Telehealth pathway: Several EU-wide telehealth platforms serve German patients for GLP-1 prescribing. The Body Clinic Germany is one Germany-specific provider. Consultations are private; prescriptions can be filled at any Apotheke.

Domestic research-chem sellers: Not applicable for legitimate trade. Sellers offering 'semaglutide research vials' are operating in counterfeit or diverted-product territory.

Underground use: Significant grey market via social media groups and unlicensed websites for compounded or non-EU-sourced semaglutide. Documented by Euronews. BfArM continues counterfeit investigations.
